Talk to Psychiatry on Memory Issue

I am suffering from depression and anxiety both. My mind goes blank when anxiety goes beyong limits and I cant recall things nor can I concentrate on anything. I am losing motivation. My university is going to start from the very next month and I dont know how am I going to study with this mindset. I am doing internship in the same uni for six months and my impression on my Head of department is not very good. She doesnt like me for no obvious reason and even insults me and humiliate me on minor issues. I dont know what to do.
