W.a.s you need posture correction exercises and time depending upon how much you doing exercise
There are multiple factors that can cause a poor posture, like your occupation, bone structure, and even your resting position of your jaw.
.To determine the exact cause of your poor posture and assess the associated muscle imbalances, you should get a complete evaluation and a customized plan from a nearby Physiotherapist.
And the time it will take to get an optimal posture can only be determined after the extent of problem is identified.
Hope it helps.
W.S.A 1st physical Examination is necessery Visit best physical Therapist Clinic then
Use Posture Belt
& Do the postural Exercises
there is a need for proper assessment of your posture muscles by a physiotherapist. time and sessions depends upon the condition of muscles.
Do this exercise 15 rep trice a day
If poor posture is related to muscular it can be correctable and you have to consult to your physiotherapist for different postural correction exercises... and you must also need to your life style like poor sitting or standing habits...
if it related to skeletal i.e bone deformation then you must consult to orthopedics for alignment correction..
if you don't correct it on time, it will also cause back pain in future. So visit a good therapist near you as soon as possible... looks like it requires 2 week sessions at least...
not complete but 50 - 60% ... for remaining, u will have to stick with long term exercise plan...

go for proper physical assessment first
secondly, do postural exercises
use posture corrector belt
avoid bad posture positions ( described by ur physiotherapist after check up)
and regulate your body activity.
first of all use posture correction belt or support. and then less use of phone plus proper treatment from nearby physiotherapist
it depends upon from how much time you are having this condition
