Asking for Self, Female, 65 years old, Lahore
She is on insulin Humulin 70/30 morning 38 unit evening 18 unit but now a days she is having fluctuation before meal Fasting glucose level is 39 and then after meal 370 before lunch again 30 now she is not taking any dose of insulin ...because before any meal sugar level is Low....
Plz monitor fbs for 7 days and get HbA1c checked
get your hba1c level checked and check your sugar level from another glucometer

I would suggest firstly good diet control with fixed timings, unless ofcourse she feels dizzy or fatigue.
Start with regular instead of humulin for time being and as she is old please get her renal profile such as urea/creatinine checked as if her kidney is effected then it will effect sugar levels also.
You need to have expert opinion of endocrinologist and Internal Medicine as there are two effect of insulin that may occur
either due to inadequate diet
due to inadequate insulin administration
