Talk to Dermatologist on Nail Surgery

Hi... My brother have nail problem since 3 yrs the thumb of foot he have done surgery of nail (half nail was cut of) and than after a year it again started to swell and pus in it blood etc and than again he gone through surgery in this May the whole nail was removed... And now the nail again started the same cycle of blood pus etc... Wanna ask what to do now again surgery.. And this is our family diseases.. Please guide and the the pic is attached. Thank you
