My son will turn 6 years old in October. His primary central incisors have fallen out and now he has four permanent teeth. The upper two central incisors are falling out. They are his permanent teeth. Why are they falling out? What should be done?

Need to take xray as your history is incomplete to decide about the cause and treatment .... have to see the patient clinically and take radiograph and then decide the treatment

at this age primary teeth shed. u might have not notice properly. u should consultant a dentist in case of any confusion
At age 6, it is definitely the primary or milk teeth falling out which would be replaced by permanent teeth. Confirmatory radiographs can be opted with your dentist if there are still any concerns.

for confirm diagnosis either share the teeth pic if possible or get the OPG xrau done n share that.

Yes the upper milky teeth start falling after the age of 6 to 7 but sometimes it can fall early as well.. no need to worry you can just get a regular check Up done to see whether the erupting teeth are doing alright or they don't have any kind of obstruction
aoa generally the permanent teeth don't fall. u should get an appointment with dentist n go for proper checkup
