Talk to Urologist on Erectile Dysfunction And Premature Ejaculation

I'm about to get married in a year, but I don't know where my problem resides. Masturbating since 8 years, gradually saw decline in my sexual health. It's being hard for me to get erections now, no morning wood. Hardly get erections but if I get one suddenly it fades away as soon as I put my hands off from it. Sometime after Masturbation had experienced burning pain under head of my penis and swelling of shaft as well. After Masturbation suddenly loose erection with in 10 seconds also sometimes I loose erection during Masturbation and had to discharge while being not hard. Its also bent towards right. Looking forward to see if anyone can help here. And in about to get married in a year and I don't want to feel ashamed in front of her so before this happens I want to evaluate and checked myself too.
