person-avatar

Asking For Other, Male 4, Jacobabad

The child was born as a twin. one girl and a boy. The girl is totally normal, walking, talking and interacting like anyother normal kid. But the boy is experiencing fits(shocks) and hits his head in front suddenly to any surface in front of him. Moreover, the boy cannot walk properly depsite normal bodily growth. He doesn't interact much and doesn't talk as well. The boy is has been taking treatment neurologists from AKUH since the last 3 years but isn't getting any better. Can any Dr here give any hint as to what is possibly causing it and what should we do.

Second borns babies in a twin delivery are at risk for suffering from hypoxia (so called lack of oxygen) at birth. This might be the cause of developmental delay and fits in your child. You did not describe his fits in detail; whether he loses consciousness or otherwise in a fit, are there jerky movements of limbs or not etc. Besides control of his fits with medicines, he needs physiotherapy to achieve his gross motor growth potential. He might also need audiometry to assess his hearing before a speech therapist might help him. He might also need services of a child psychologist for behavioural therapy. Such services are supposed to be available in Developmental Paediatrics Department of a Children Hospital. Please, check a government sector children hospital in Karachi for that if there is any.

As to the nature of fits, he gets them in his head. for instance, while standing on the support of a table, he suddenly bangs his forehead on the table and while standing (he hardly stands for more than a minute on his own) he suddenly falls on his face. Just like when someone suddenly falls asleep while sitting or standing, one loses control of ones head.

The best way to report a fit to a doctor is to videotape it. From how you describe it, he seems to have Atonic Seizures. However, a video recording of the whole episode will be helpful in determining the nature of the seizures.
