Talk to Pediatrician on Milk Teeth Extraction

My 6 year old's milk teeth have not fallen out and the new teeth have started growing behind them. 2 of them. The front incisors. The milk teeth are only moving very slightly. The dentist has advised to remove the milk teeth but from my experience the injection and stuff are quite painful. Do we have any other options? Pictures attached
