Asking For Father, Male 65, timergara, khall
my father had brain stroke and he is unconcious since 30 june till date i would like to consult a doctor for a better treatment though he is under treatmentment but no changes in his condition..so kindly drs i need consultant for best treatment...MRI report shows that he has old infract in perito occipetal lobe and a small infract in right MCA including cortex ...
Your father needs nursing care and rehabilitation only, continue whatever treatment is going on
any co morbidity? diabetese? blood pressure on any cardiac problem?

yes he has already diabetes, heart problem, and a minor brain attack 2 yrs before .

This is his 1st stroke or Had strokes in the past.
Any other Co-morbids like Hypertension, diabetes,Ischemic Heart disease, Renal failure, COPD/ASTHMA,...
What do you mean by unconscious.
What is the treatment currently he is taking.
What Ejection Fraction on 2D-ECHO.
Any active Infection now.
Any instrumentation like Folay,Nasogastric tube.

thanks , yes he has already diabetes,and heart problem is well and also had a minor brain attacked 2 years before ..
by unconcious i means he can't speak and see and his left side is paralysed.
At first 10 to 20 days he had chest infection but by using tanzo 4.5 gm inj he has no chest infection currently, but has TLC was 16465 before a couple of week .
he has engitube and urine tube .
in his medication tab ascard plus, syrup serib, mosegar, ensure powder prescribed by dr and said to give him nursing care and exercise .

It will take time for him to recover at least 3 to 6 months.
Things delaying his recovery
1:Aspiration Pneumonia
2:Urosepsis due to Prolong Catheterization
3: Bedsores
4: Constipation
5:Stroke off effects
6:Metabolic derangement like Blood sugar, Urea, creatinine
Things to do change Folay/NG tube to Silicone and Try to change every month.
Hourly posture changes, Air Mattress, Releive constipation, regular limbs & chest physiotherapy, Daily BSR, Fresh CBC, Urine C/C, Ammonia, lactate.
Paralysis,Unable to speak..... These issues will take time to settle.

There could be a number of reasons for him not being able to gain consciousness. In the setting of stroke most common is stroke in the area responsible for consciousness followed by second most commonly an underlying infection. Thirdly, an underlying seizure which is only happening in brain and not visible to eye is also possible. A failed l detailed in person exam and labs are needed to reach diagnosis
