First of all kindly get CT KUB (without contrast) done with UCE.
as we need to see the cause of pain? as stone of 5mm in size should not cause that much severe pain on right side, even 1 cm stone is too not much convencing
By CT exact location of the stone could be localized, stone ( itself or any other cause) is putting pressure on the kidneys that may result in the reduction of functionality or severe infection. Treatment would depend upon the cause whether its a particular site which is responsible for pain by a very small stone and compressing it? only after that one can decide about the outcome and benefits of URS versus Lithotripsy . Other causes infection ? stricture? and most importantly the current status of the kidney function ? how much it has got affected uptill now? All these factors needs consideration for treatment.
