Aoa sir Sir I am living in bishkek in kyrgyzstan my age is 20 sir, about 3 months ago I feel chest pain and shortness of breath i consult to a cardiologist here in kyrgyzstan my ecg and echo was normal but I am feeling the pain he said there is no heart problem more ever he suggested me to test for asthma which was also normal After that I didn’t any symptoms then about 2 weeks ago I started feeling chest pain with heart palpitations I consult to a cardiologist again ecg is normal he gave me a medicine mydocalm and calm heart but I don’t feel so much relive in addition I started feeling pain around my jaw probably in mandible bone and chest pain radiates to neck it’s something like someone suffocated me it lasts about 5 mins Whenever I walk for a long time or do some difficult physical work I feel same pain and it’s feels like unconsciousness and lightheaded which will go away with rest factors: I don’t smoke a single time in my life not alcohol I have no diabetes my weight is

W Salam, thanks for your question.
The symptoms you mentioned can occur in stress related disorders, Thyroid hormone abnormalities so TFTs should also be done.
A normal ECG and Echocardiography rules out common cardiac disease, however if someone still remain symptomatic, we sometimes do other cardiac investigations including 24 hours Holter monitoring and CT Coronary Angiography.
Gastritis and GERD can also lead to similar symptoms.
Therefore, I would recommend you should discuss with your doctor to do above mentioned investigations.
A trial of PPI like Omeprazole for 4 weeks can also be helpful.

And sir my age is only 20 weight is 70 kg with height of 5.7
Is there a chance of CAD ??

In young age very less chances of CAD , however in patients with strong family history or heavy smokers , CT Coronary Angiography can be done.
Because you feel palpitations, light headedness and transient unconsciousness, 24 hours Holter monitoring should be done.
Meanwhile try to control your anxiety with low dose anxiolytic, counselling sessions by Psychologist.
PPI for 4 week period can be useful.

Sir due to lockdown I was late for test
My report is in russian language so I translated it
Holter
Conclusion: Total complexes 042-32591427, . Sinus rhythm. Sinus arrhythmia. Average heart rate 87. max. Heart rate 164 Sun 13:51. Min. Heart rate 51 at 06:43. ZhES-no. SV es-108- isolated, 3 group atrial es, (4-5). 2 episodes of atrial ectopic rhythm at 07: 49: 40s heart rate 80 at 07:49:40 prod. 7 sec, 09:35:09 with heart rate 82 prod. 7 sec. There are no episodes of ischemia.
Ett is stopped bcz of high bp 169 and heart rate 174 Though ETT is negative

If your recent ecg and echo are normal then no need to worry but the symptoms u are describing are typical of ACS. Consult another cardiologist if possible .. it can also be Panic disorder ( Anxiety disorder ). For time being take Tab. Inderal 10mg at morning and Tab. Alp 0.5 mg at night .. let me know after one week of treatment . Regards

completly agree with Dr wajahat Bangsh. HOLTER monitering should be done to exclude cardiac arrythmias with considering Anxiety disorder.
