Yes the thalassemia trait is confirmed. However this is an asymptomatic condition.
We expect to have Hb of 10 max with trait.
Reports show concomitant iron deficiency as well. In a 4 years old child if you are in Pakistan you can get syp.maltofer 1 tsp 3 times a day. Or iron drops which u can include in food. Usually 3 months treatment is fine but you can keep giving it even after that.

Iron deficiency in child is due to increased growth requirements and poor diet. I also suspect worm infestation. Syp.Vermox 1 tsp for 3 days OR syp.zentel full bottle just once.

Liver and dates are rich sources of Iron.

Also genetically its important ur child doesnt marry someone with thalassemia trait as there would be 25 to 50% chance of having thalassemic child

Greetings from Pakistan.
A.s.a.. since we don't know exact weight of the child we assume it to be 15kg.. iron replacement should be as follows..
Inj. Ferrinject, 500mg I/V as single dose
Followed by
Syp. Maltofer, 1+1
Tab. Folic acid, 5mg, once daily
Both for 03 months..
