he should do light yoga or walk 3 times a day, quit smoking if he's doing it, massage with vicks vaporub.

These are signs of diabetic neuropathy.
This shows either he is not following the drug regime well or the doses need adjustment.
Record daily fasting blood sugar levels
And blood sugar levels 2 hours after meal.
According to this record insulin dose will be adjusted.
Regarding symptoms please do a light yoga, observe cleanliness, wear socks, do massage.
Quit smoking
Plus if a person is on insulin he has to be very strictly use the drug for life. As you have mentioned sometimes it does improve with insulin
This shows that the patient quits taking insulin when he feels better. It should not be the case.
