Patient might have Urinary Tract Infection as there is presence of Organisms in Urine Examination. ESR is also suggestive of Ongoing Infection. For Further Evaluation get Urine C/S Done. Most Common Cause of Decreased Hb is Iron Deficiency. For Further Evaluation get Serum Ferritin Done.
Furthermore do share Brief History of Patient for Proper Diagnosis & Treatment.

General Practitioner in Karachi - Dr. Shaima Amjad

Hello. Can u give a proper and detailed history of her.. Also her current symptoms and why was she advised to have all these tests.
According to the reports, she may have Urinary tract infection but we need a urine culture test to confirm it. And if that comes positive, then we need to prescribe antibiotic. Till then if she's having burning in passing urine, or increased urine frequency then she can use cranmax sachet, 1 in morning and 1 at night.

Blood report CBC shows Anemia. Most likely it's secondary to Iron deficiency but we need Iron profile and folate tests. ESR is raised which indicates some sort of chronic inflammation. History may help towards accurate diagnosis.

if you're comfortable and need to discuss further, we can have chat on Marham or u can contact on my number which is 042-32591427, 81

Rest of the tests appear normal..
