I have anal fissure (as disgnosed by a physician) since 2 years and i used sitz bath and cream mix for 2 weeks but not cure properly and also have a little lump mass due to fissure, i am worried about this, plz guide me what i have to do

hithanks for asking a question. Most common cause for developing anal fissure is chronic constipation (passing hard stools associated with straining). Key is to avoid constipation use faecal softner in form of laxative like movicol/lactulose etc more so healthy diet with increased fibre and fluid helps.for pain rel use either topical anesthetic like lidocaine /xyloproct if this fails you can use GTN 2% OINTMENT LIKE RECTOGESIC  twice/day this will relax external sphincter muscle to relieve spasm, this may cause a headachewe also use diltiazem topical cream some time but I believe it would be very expensive in Pakistan.if all these fails then the examination is needed to ensure no sec cause like inflammatory bowel disease is underlying cause very rarely it can be associated with other illnesses.some time the surgeon offers botox therapy but this can cause possible faecal incontinence.the surgical option is fisherotomy which is done under general anaesthetichope this is helpfulif you need to a consult or further discussion you can book online consult regardKashif
