There's not much I can assess with the complaints of pain which I see is all over the face and shoulders and neck. Xray PNS is clear. About pain... continuous or comes and goes? When increases and how it is relieved? Any nasal complaints like runny or stuffed or block nose? Any throat pain with fever? Dental issues? Any habits of Bruxism (grinding teeth)? Any eye related complaints?? What a out his general condition and behaviour? Any medical illness he's having? What meds he has used and is currently using? Is he using any analgesia (pain killers right now? if yes, what and how much? After all these queries have been answered only then a possible workup or treatment can be suggested.

Your nephew is having sinusitis. You didn't mention his current symptoms. He needs to take medication and precautionary measures regularly. Right now if he has blocked nose, facial pain and fever then he should start
