Aoa My uncle is a sugar and heart patient, recently had a heart attack and had angiography 4 times with stents. I have been monitoring his sugar levels on glucometer. As for fasting it's near 200 or above. And 2 hours after meal it's 250-300. His current insulin related dosage is: Novorapid: 12 thrice a day Lantus: 30 once at night time

You have to monitor 2 or 3 days more...if persistently FBS and RBS levels may increase than u have to increase the lantus unit as well as novorapid

lantus 34 unit at night...monitorinf should b on regular basis
Check HbA1c. Up titrate lantus step-by-step according to fasting sugar. As our main meal is usually lunch so Novorapid can be up titrated by 2 units at lunchtime. If not controlled consult endocrinologist regarding adding OHG. Do echo before consultation with endocrinologist as he may decide the best suited oral hypoglycaemic for the LV function.
Tab. Jardy 25mg at night before meal... And continue these insulin...
Repeat HbA1c level
Take medicines of heart doctor as prescribed. Increase Novoralid to 14 units, rest same. Monitor sugar 2 hours after meals. If it drips below 100, don’t give insulin of thst time.
