Talk to Cardiologist on Cardiology
Asking for Self, Female, 26 years old, Peshawar
Assalamoalikum I want to ask any cardiologist here about my problem. M 26 yrd old I have been feeling chest tightness and heaviness on exertion. Did ett it was normal achived percent thr but was very out of breath. The symptoms still persist. A consultant cardiologist suggested to do ct cor angio . kindly tell me should i proceed with it? I fear the radiation n contrast dose might be very high on ct angio. I have heard that 640 slices ct scanners uses radiatiom even lesser than the traditional angiography. Kindly guide me in this regard that what should i do. I cant consult my cardiologist due to corona lockdown.

Aoa. given your age, sex and ETT negative there is no indication for a CT coronary angiogram.
Has your doctor done echo for you ? If not do it. MIBI SCAN Is alternative to CT cor angio but the good point with CT cor angio is that if it comes out to be normal, one can be 99% sure that you don’t have disease in the heart vessels.

only ca scoring is enough on CT. full CT not required.
Some distortion of text occurring plasm marham to sort. Eg the words don’t
If ETT is normal, and with your age. No need to go for CT Angio. Would suggest you to take a Pulmonologist on board to see lungs as cause of chest tightness

Waa alkyum Salam Do u have any risk factor like HTN ,DM Family history,raised cholesterol etc?if NO then chance of CAD is low.i suggest chest X-ray,trans thoracic echo.if all normal you can undergo stress Echo,safe from radiation with reasonably good sensitivity and specificity.

WAA. If you have no FH of early CAD, and if you are non smoker than there is low likelihood of CAD. But if you symptoms persist, get your chest examination for asthma and do spirometry, Cxray. If these are ruled out than needs to assess further cardiac investigations.

Sir i have never been told by any Cardiologist/ medical specialist/ endocrinologist to go for serum aldo and plasma renin. They just look at eIelectrolytes lvl. wanted to do that too but as i m using arbs its not possible for me to do it. Now i have also noticed from few days that when ever theres stressful situation or when i m doing stressfull work i feel pain in the middle of the chest sometimes at rest too in stressful situations. No other symptoms along with pain

If you are not obese, Heart rate is between 60 to 70 and Sinus rythum, CT Angio result will have good sensitivity and specificity. Rule out secondary causes of Hypertension in this age group that's why Aldosterone and Renin level but you have to b off from ARBs and ACEI definitely.

With hx of htn borderline lvh and exertional symptoms ccta will rule out coronary occlusion if negative. The radiation exposure will be less than 3 mSv. Radiation from CXR is half a mSv

fine. That is quite an extensive work up done already. I don't think so right now any further testing is required to rule out coronary artery disease. what is your BP readings these days ?

Hypertensive patient, exertional symptoms and dyspnea during ETT. ETT has 70% sensitivity for picking coronary heart disease. It misses out 30%. Better to have MIBI OR CCTA. Should stop bblocker for stress MIBI.
